Pregnant women whose pregnancy reaches the fourth month or more will benefit. These include those who had premature births, stillborn babies and miscarriages.

For the pregnant woman to benefit, she must be the holder of the insurance or be a dependent of her spouse enrolled in the health insurance, Shakai hoken or kokumin kenko hoken. If the pregnant woman or her spouse is registered with shakai hoken, she must ask the company to submit the request. If the pregnant woman or her spouse is registered with the kokumin kenko hoken, it must be requested at City Hall.

Benefit amount: 390,000 yen or 420,000 yen

How to request

When giving birth in maternity hospitals that run the direct payment system

  • When the insured or their dependents sign a written agreement with the maternity hospital that runs the direct payment system, the birth expenses will be paid to the maternity hospital within the Maternity Assistance (Shussan Ikuji Ichijikin) limit.
  • When the cost of childbirth is less than Maternity Assistance (Shussan Ikuji Ichijikin), the difference will be paid to the insured. You must submit the Request for Partial Payment of Maternity Assistance/ Maternity Assistance for Family Members/ Request for Difference in Birth Expenses (Shussan Ikuji/ Kazoku Shussan Ikuji Ichijikin Uchibaraikin Shiharai Iraisho/ Sagaku Shinseisho) within 2 years from the day following the day of childbirth.
  • You will need to attach to this Application, proof from the doctor, midwife or Municipal Mayor and a photocopy of the written Agreement (Goibunsho) regarding the direct payment.

 When giving birth in maternity hospitals that do not implement the direct payment system

  • You must submit the Application for Payment of Maternity Assistance/ Maternity Assistance for Family Members (Shussan Ikuji Ichijikin/ Kazoku Shussan Ikuji Ichijikin Shikyu Shinseisho) within 2 years from the day following the birth. The Application form must have the supporting field filled in by the doctor, midwife or Municipal Mayor and attach a photocopy of the receipt for birth expenses issued by the maternity hospital specifying that it has not used the direct payment system.

When giving birth outside of Japan

  • You must submit the Application for Payment of Maternity Assistance/ Maternity Assistance for Family Members (Shussan Ikuji Ichijikin/ Kazoku Shussan Ichijikin Shikyu Shinseisho) within 2 years from the day following the birth. You will need to attach the Medical Birth Certificate (Shussan Shomei). In addition, you must attach the Declaration Form indicating that you are not receiving Maternity Benefit twice, if the insured person gave birth within 6 months after voluntary dismissal, or if her dependent gave birth within 6 months after have become dependent.
