The city of Inazawa, with the aim of contributing to the improvement of children’s health and well-being, offers subsidies for medical expenses not covered by health insurance.
Request for medical expense benefit certificate
Take your child’s eligibility documents for the health plan (eligibility certificate, eligibility information notice, etc.) and request issuance at the Municipal Health Insurance and Pensions Division, at the Sobue/Heiwa branch office, or at the Community Center.
Expiry date of the child medical expenses beneficiary certificate
- Preschool children: Until the end of the fiscal year in which the child turns 6 (the first March 31st after the child turns 6).
- Elementary school students: Until the end of the fiscal year in which the child turns 12 (the first March 31st after the child turns 12).
- Middle and high school students: Until the end of the fiscal year in which the person turns 18 (the first March 31st after the day they turn 18).
If there is out-of-pocket payment for medical treatment outside of the city hall, etc.
The “Children’s Medical Expense Beneficiary Certificate” cannot be used for medical treatment or the purchase of assistive devices at medical institutions outside of Aichi Prefecture. Therefore, pay the medical expenses at the institution where you received the treatment.
Afterward, submit a medical expense reimbursement request to the National Health Insurance and Pension Division of the City Hall, at the Sobue/Heiwa branch office, or at the Citizen Center, attaching the following documents for each month of treatment and medical institution.
